COA Volunteer Application
The South Hadley Council on Aging offers a wide variety of volunteer opportunities and we are very fortunate to have so many loyal volunteers. Fitness Center Application
To access the Maurice "Fitz" and Margaret Fitzgerald Memorial Fitness Center at the South Hadley Council on Aging, members must complete the application in full, including the participant disclaimer form, which outlines liability policies. Participants are strongly encouraged to consult with their physician or healthcare provider before beginning any fitness routine. Once your application is approved, you will be scheduled for a mandatory orientation before gaining access to the Fitness Center. To download a current application, click here.
Lock Box Application (District #1 and District #2)
Residential Emergency Lock Box Program through TRIAD Council enables seniors to have an emergency lock box that holds their house key. The lock box is installed by the Fire Department who also hold the Master Key. Return completed application and payment to the Council on Aging. For an application, click here.
