Access Care Partners

Formerly known as WestMass ElderCare, Access Care Partners (ACP) is a private, non-profit organization that has been serving the community since 1974. As South Hadley's designated Area Agency on Aging, ACP is funded through grants and contracts from the Massachusetts Executive Office of Aging and Independence.

ACP is dedicated to empowering older adults, individuals with disabilities, and caregivers by providing compassionate, personalized care solutions. Their mission is to connect people with the resources and services they need to maintain their independence, dignity, and connection to their community.

Guided by a vision of health equity and inclusion, Access Care Partners is a trusted leader in promoting independence and supporting the well-being of all individuals, regardless of age or ability.
